Program Description

  • Adult Protection
    To report suspected abuse, neglect, self-neglect or financial exploitation of vulnerable adults, call the Minnesota Adult Abuse Reporting Center (MAARC) 844-880-1574 or http://www.mn.gov/dhs/reportadultabuse/. Reports are referred to local social workers that take appropriate action to provide safety.
  • Adult Foster Care
    Adult Foster Care is a licensed home-like setting for adults who are unable to live on their own. Placements may be short-term, long-term or for respite care.
  • Blood Pressure Checks
    Public Health Nurses perform blood pressure checks for elderly persons. Appointments are required.
  • MNCHOICES
    Web-based application that is comprehensive and integrates assessment and support planning for Minnesotans who need long-term services and supports. MNCHOICES embraces a person-centered approach to ensure services meet each person’s strengths, goals, preferences, and assessed needs.
  • Services to Seniors
    Assessments and referrals to assist individuals age 65 and older to remain in their home, as well as recommendations for options outside of the home when necessary. Common services that people access include homemaking, nurse visits, medical alert systems, home delivered meals and transportation.
