Notary Commission Registration

To become a Notary Public in Minnesota, to obtain notary public forms, or to obtain any other reference materials, contact the Office of the Minnesota Secretary of State at:

Registration of Notary Commission

The Minnesota Office of the Secretary of State will issue a Notary Commission card. This card must be registered with you resident Minnesota county.

Register in Person

Please bring:

  • The notary commission certificate that you received from the Secretary of State
  • $20.00 recording fee
  • Government-issued photo ID (valid Driver’s License or US Passport)

Register by Mail

Please enclose and send:

  • The notary commission certificate that you received from the Secretary of State with your signatures completed on the appropriate lines, one reading exactly as your name is spelled out on your notary commission certificate and the other signed the way you commonly sign your name and/or how you will do so in performing notarial acts.
  • Include a complete Affidavit of Signature Identify for Notary Public Commission form (download below) with the appropriate signatures as defined above which must be notarized (by a different notary).
  • $20.00 recording fee

You may pay be cash or check; please make checks payable to Lake County Vitals.

After your commission is recorded, we will return the commission certificate to you with paid receipt and proof of filing in our office.
